Personal Instruction Form

Your Personal Instruction Form (PIF) is an important record of your wishes for end-of-life. It contains information needed for a death certificate. In it you'll record where body disposition will take place, the kind of memorial services you would like, and family or friends who should be contacted at the time of your death. This takes stress and worry off the shoulders of your survivors - it's a gift to them and ensures that your wishes are carried out.

​The form is digitally fillable, so you can update it any time and save it as a digital file. You can also print it. Complete the form and keep it in your files.

We recommend giving this form to a mortuary ONLY when you are actually entering into a contract or pre-paying. The information on this form, including your social security number, is best kept secure and with your other end of life directives and information. Your OMA member number is at the top, should it be needed to access OMA discounts. If you have a preference for a specific mortuary, you might visit and let them know you are an OMA member. We no longer recommend giving the PIF as a preliminary interest form.

  • OMA no longer stores your completed PIF, just your choice of mortuary, body disposition preference, and survivor contact information. 
  • If you would like your choice of mortuary, body disposition preference, and survivor contact information recorded in your OMA member record, contact us at  omaoregon@gmail.com or 503-647-5590.
  • Please do not mail your completed form to OMA.

State and Federal Assistance
           Federal Emergency Management Agency (FEMA):  For deaths as 
           a result of an event that was declared a major disaster or
           emergency (including COVID-19 related deaths), you may qualify
           for FEMA Funeral Assistance to help with expenses such as a
           casket or urn, burial or cremation, and the cost of a maker or
           headstone.

           State government assistance:  Many states have programs to
           help if you cannot afford a funeral or burial. In states that have
           programs, you’ll have to apply for assistance at the county or
           municipal level. Here is a list of programs by state, compiled by
           Funeralwise.

           Veterans benefits: If you are a veteran, you can be buried in one
           of the 141 national cemeteries at no cost. Spouses and children of
           veterans could also qualify. Check with the
           US Department of Veteran's Affairs for eligibility requirements.
